Application for Registration - Meat and Poultry
Licensed facilities include fully inspected meat plants - those which can sell directly to the consumer, hotels and restaurants and retail stores within Kansas; custom plants - those which process meat for the owners, their families and guests; wholesalers/distributors; animal food manufacturers and brokers.
Wholesaler or Public Warehouseman Registration
Wholesaler, Distributor and Public Warehouseman is required to register with Kansas Department of Agriculture before they engage in business. You will need to list all persons, individuals, partners, officers, holders or owners of 10 per centum or more of voting stock, and employees in a managerial or executive capacity in the business. To obtain label approvals, you must contact the inspector at the plant where the animals are processed.
Wholesale Form
This is a supplement form to the Application for Registration form. Please indicate what slaughter/processing facility that you use.
Meat and Poultry Custom Form
If you checked any of the Custom fields on the Application for Registration, you will need to fill out the Meat and Poultry custom form.
