Meat and Poultry Custom Form
If you checked any of the Custom fields on the Application for Registration, you will need to fill out the Meat and Poultry custom form.
Meat and Poultry Registration Wholesaler
This is a supplement form to the Application for Registration form. Please idndicate what slaughter/processing facility that you use.
Meat & Poultry License ApplicationLicensed facilities include fully inspected meat plants - those which can sell directly to the consumer, hotels and restaurants and retail stores within Kansas; custom plants - those which process meat for the owners, their families and guests; wholesalers/distributors; animal food manufacturers and brokers.
An applicant for a license must provide all information requested and pay an application fee based on the number of animals slaughtered. Only the person who is named on the license can conduct operations at that licensed establishment. Persons interested in entering this business should contact us. Our review team will visit your establishment and make suggestions to meet requirements necessary to become licensed and begin operating. Licensure expires December 31.
Wholesaler Application
Wholesaler, Distributor and Public Warehouseman is required to register with Kansas Department of Agriculture before they engage in business. You will need to list all persons, individuals, partners, officers, holders or owners of 10 per centum or more of voting stock, and employees in a managerial or executive capacity in the business. To obtain label approvals, you must contact the inspector at the plant where the animals are processed. There is no fee and the licensure expires December 31.
