Form 1-204.5 (Certification of Water Flowmeters) must be completed and submitted to the chief engineer with appropriate supporting material. Supporting material could include National Institute of Standards and Technology (NIST) documentation, meter specification sheets, a photo or plan drawing of the register seal, and explanation of how the flowmeter is designed so that the meter will be accurate throughout its operational life. The form should be signed by someone in the company with direct knowledge of quality control and testing performed on the meter. A technical services staff member will review the form and documentation. If the form is incomplete, or there are questions about the flowmeter, the staff member will contact the person listed on the form. Once the staff member is satisfied that the flowmeter model meets the specifications of the chief engineer, a recommendation and draft acceptance letter are forwarded to the chief engineer. The chief engineer reviews the recommendation and, if he agrees, he signs a letter of acceptance. The letter is sent to the contact person listed on the form and the model is added to the certified flowmeter list.
